A parent/legal guardian may submit an entry on behalf of his/her child who is under 18 years of age. Employees of Sponsor and its respective affiliates, subsidiaries, parent companies, sales representatives, agencies, dealers, retailers, wholesalers, distributors, and promotional and vendor agencies involved in the Contest, and the immediate families (spouse, parents, siblings and children) or household members of all such employees, are NOT eligible to win. The Contest is subject to all applicable federal, state and local laws and regulations. Void where prohibited or restricted by law or regulations. Participation constitutes entrant's full and unconditional agreement to these terms and conditions and Sponsor's decisions, which are final and binding in all matters related to the Contest.
ENTRY PERIODS / HOW TO ENTER: The Contest runs from July 6, 2017 to 11:59 P.M. CT on October 6, 2017 (the "Contest Period"). The Contest consists of three monthly entry periods (each an "Entry Period"): July 6 – August 6 (Entry Period 1); August 7 – September 6 (Entry Period 2); and September 7 – October 6 (Entry Period 3). A monthly winner will be selected from the entries submitted during each Entry Period. In addition, one Grand Prize winner will be selected from among the ten (10) highest scoring entries received during the Contest Period, as described below.

How to Enter: To enter, please visit AmputeeLifeHacks.com during the Contest Period and complete the steps of the online entry process:
1. Complete the on-line entry form: You will be required to provide your full name, email address, phone number, age, and home address for purposes of Contest administration.
2. Upload a "Amputee Life Hack" Video. Submit a video that is three (3) minutes or less in length showing a creative technique that you have developed to accomplish a specific activity or task. Examples may include tying a shoe with an arm prosthesis, styling hair using residual limbs, or any other creative technique or accommodation that you have developed or would like to share with similarly situated individuals. Please see below for additional video submissions guidelines.
3. Provide a Written Description of the "Amputee Life Hack." Provide a written description of the amputee life hack portrayed in the video. The description must be 150 words or less and must be written in English.
Entrants may submit as many entries as they wish so as long as each entry demonstrates a unique technique or accommodation and complies with all other terms and conditions outlined in these Official Rules.
Video Submission Guidelines: The following guidelines apply to videos submitted in connection with this Contest.
•Entry must be no longer than 3 minutes in length.
•Entry must be in one of the following video formats: .mpeg, .mpg4, .avi, .mov, .flv or .wmv.
•Entry cannot exceed 500 MBs in size.
•Entrant may show entrant or his/her child/legal ward. If any other individual is shown in the video, entrant must obtain consent from that individual prior to submitting the video, and, if requested by Sponsor, entrant must provide to Sponsor written consent from individual(s) shown in the video.
•Entry must be entrant's original creation and may not include copyrighted, trademarked, patented, or previously published material. Entry requiring license or usage rights from any third party is not eligible. Entry may not have been submitted previously in a contest of any kind or previously exhibited or displayed publicly (i.e., disclosed beyond your immediate circle of friends, and family) through any means.
•Entry must not (and entrants represent and warrant that they will not) violate or infringe any third-party copyright, trademark, trade dress, or other proprietary right of any entity or person (living or deceased), including but not limited to rights of privacy, publicity or portrayal in a false light.
•Entry must not make reference to any commercial/corporate advertising, including but not limited to corporate logos, brand names (other than Sponsor's), slogans, political, or religious statements, or be otherwise objectionable, as determined in Sponsor/Administrator's sole discretion.
•Entry must not include any representation of celebrities, athletes, musicians (other than Artist), or any other public or private figure living or dead, or include any anti-social, political or religious groups, any commercial solicitation or promotional materials or address, phone number, or URL address.
•Entry must not contain material that promotes bigotry, racism, hatred, or harm against any group or individual or promotes discrimination based on race, sex, religion, nationality, disability, sexual orientation, or age.
•Entry cannot defame, misrepresent or contain disparaging remarks about Sponsor, the Released Parties, or their respective products and services, or other people, products or companies.
•Entry must be consistent with Sponsor's reputation in the marketplace and must be suitable for audiences of all ages. Entries which, in Sponsor's or judges' sole discretion, contain any type of vulgarity or objectionable content will be disqualified.
•Sponsor reserves the right, but not the obligation, to modify, blur images, or otherwise make modest edits to an entry so that the entry conforms to the stated video submissions guidelines.
Entries that do not meet these stated criteria, or entries that (in Sponsor's or judges' sole discretion) may technically meet the stated criteria but do not reflect the intent and the spirit of the Contest, will be disqualified.
Entry constitutes entrant's certification that (i) s/he is the owner of the entry; (ii) is able and authorized to submit it; and (iii) has received the consent of anyone shown in the video to submit the entry. By submitting entry, entrant agrees to indemnify and hold harmless Sponsor, YouTube, and its parent companies, subsidiaries, affiliates, sales representatives, agencies, retailers, wholesalers, distributors, and promotional and other vendor agencies involved in this Contest from any and all claims, damages, expenses, costs (including reasonable attorney fees) and liabilities (including settlements), resulting from or relating to entrant's submission, or any information supplied or represented to Sponsor by entrant.
This Contest is in no way sponsored, endorsed, or administered by, or associated with, YouTube or Facebook. By entering, you understand that you are providing your submission to Sponsor and not to YouTube or Facebook.
By submitting an Entry, you understand that if your Entry is deemed a Finalist (defined below), it will be posted at AmputeeLifeHacks.com for members of the general public to view and to vote on and that any such posting will be deemed made at the direction of the Entrant within the meaning of the Digital Millennium Copyright Act and the Communications Decency Act.
SELECTION OF PRIZE WINNERS: Sponsor will judge the first 250 eligible entries received during each of the three monthly Entry Periods. In the event that Sponsor receives 250 qualifying entries in a particular Entry Period, Sponsor will post a notification on the Contest website, www.AmputeeLifeHacks.com, stating that the maximum number of entries (i.e., 250 entries) have been received for a particular Entry Period.
Winner Selection for Monthly Entry Periods. Entries will be judged by a panel of judges selected by Sponsor, which may include marketing, public relations, and patient care representatives. Entries will be judged on a 500-point scale by the panel based on the following four (4) criteria: (i) incorporation of #AmputeeLifeHacks' theme (25%), (ii) technique/craftsmanship (15%), (iiii) quality of written description (10%) and (iiii) creativity/originality of Life Hack (50%).
The entry receiving the highest score for each monthly Entry Period will be declared the potential winner for the Entry Period. In the event of a tie, the tied entry that scores highest in the creativity category will be deemed the potential winner. If a tie still exists, the tied entry that scores highest in the written description category will be deemed the potential winner. All decisions by the judges are final and binding on all entries.
Potential monthly winners will be notified approximately two (2) weeks after the close of each Entry Period. Potential winners will be contacted by phone, email, or regular mail, in Sponsor's sole discretion, using the information provided on the Entry form. The potential winner will be required to respond within five (5) business days of notification, or the prize will be forfeited and an alternate winner will be chosen. Affidavit of eligibility (including SSN), prize acceptance form, tax information, assignment of rights, publicity release, and/or mutually acceptable liability releases may be required and, if so, must be returned within the reasonable specified time or monthly prize will be forfeited and an alternate winner chosen.
Winner Selection for Grand Prize / Public Voting. After judging has been completed for the last Entry Period, the top ten (10) highest scoring entries received during the Contest Period will be deemed Finalists. The ten Finalist videos will then be subject to a public-voting period for judging by the general public. Specifically, the Finalists videos will be featured at www.AmputeeLifeHacks.com on October 13, 2017 for viewing and voting by website viewers. Individuals visiting the voting website will have the opportunity to vote on their favorite Finalist video. The public voting period will end at 11:59 p.m. CT on October 27, 2017 (the "Voting Period"). There is a limit of one vote per person/email address per Finalist video per day. For purposes of clarity, an individual may cast one vote for one Finalist video each day during the Voting Period.
The Finalist video with the highest number of valid votes at the conclusion of the Voting Period (as determined by Sponsor in its sole discretion) will receive 50 additional points to his/her monthly Entry Period score; the Finalist video with the second highest number of valid votes will receive 40 additional points to his/her monthly Entry Period score; and the Finalist video with the third highest number of valid votes will receive 30 additional points to his/her monthly Entry Period Score. The grand prize winner will be the Finalist entry with the highest combined score of both (i) monthly Entry Period score and (ii) public voting points. In the event of a tie, the Finalist that received the most points in the public voting portion of the Contest will be deemed the potential grand prize winner. If a tie still exists, the Finalist with the highest monthly Entry Period score will be deemed the potential grand prize winner.
PLEASE NOTE: Any attempt to use multiple e-mail accounts or other tactics to vote more than the stated limit may result in disqualification and all associated votes will be void. Votes generated by script, macro, bot, commercial Contest subscription, incentives or sweepstakes not sponsored by Sponsor, vote-swapping sites, voting software, entering service sites or any other automated means, and votes by any means that subvert the voting process or do not conform to the spirit of these Official Rules or to the intended fair play of the Contest will be deemed void. Sponsor reserves the right to disqualify any person tampering with the voting process and to disqualify and void any votes that were submitted by fraudulent means or were otherwise in violation of the voting rules and instructions, as determined by Sponsor in its sole discretion.
Entrants may use social media or comparable means to direct potential voters to the voting website and select their entries. However, in no circumstance may compensation be offered or suggested.
Potential grand prize winner will be notified on or about November 3, 2017. Potential winner will be contacted by phone, email, or regular mail, in Sponsor's sole discretion, using the information provided on the Entry form. The potential winner will be required to respond within two (2) business days of notification, or the prize will be forfeited and an alternate winner will be chosen. Affidavit of eligibility (including SSN), prize acceptance form, tax information, assignment of rights, publicity release, and/or mutually acceptable liability releases may be required and, if so, must be returned within the reasonable specified time or monthly prize will be forfeited and an alternate winner chosen. The potential winner must maintain confidentiality of his/her status as the potential winner, and may not publicize his/her status as the potential winner, whether on social media or otherwise, until Sponsor officially announces the winner. Potential winner's failure to maintain confidentiality of his/her status as the potential winner may result in forfeiture of the grand prize, in the sole discretion of the Sponsor.
Odds of winning depend on the number of eligible entries received and the quality of the entries received for each monthly Entry Period (for monthly prizes) and the number of eligible entries received and the quality of the entries received during the Contest Period (for grand prize).
The grand prize winner of the Contest will be revealed on or about November 8, 2017 by recognition on the Contest website. The grand prize winner's video may also be shared, at the Sponsor's sole discretion, on Sponsor's social media channels and websites, including but not limited to Facebook, Twitter, LinkedIn, and YouTube.
PRIZES & APPROXIMATE RETAIL VALUES (ARV): There will be one (1) winner selected based on the judging criteria for each of the three monthly Entry Periods, for a total of three monthly winners. In addition, there will be one (1) grand prize winner selected from among the Finalists.
Three (3) Monthly Prize Winners: Each monthly winner will receive the following: one (1) Hanger T-Shirt (ARV: $25) and one (1) $100 Visa gift card (ARV: $100).
Total ARV of each Monthly Prize: $125.
Grand Prize Winner: The grand prize winner will receive the following:
•Round-trip coach airfare for winner and one (1) guest from a major domestic airport near winner's place of residence to Tampa, Florida.
•Three (3) nights hotel accommodations in one (1) standard double-occupancy room (excludes meals, room service, or other incidentals) at a hotel selected by Sponsor.
•VIP experience for winner and one (1) guest to meet Winter the Dolphin at the Clearwater Marine Aquarium (CMA), and a tour of CMA.
•$500 check to be used towards meals, ground transportation, or other travel-related expenses or incidentals.
Total ARV of grand prize: $3,000.
Grand Prize Travel Details. If winner lives within a 150-mile radius of Clearwater, Florida, ground transportation may be provided in lieu of air transportation at the sole discretion of the Sponsor, and no compensation or substitution will be provided for difference in prize value. Sponsor will make the sole determination whether winner's residence is greater than 150 miles away from hotel location, using its own methods and tools (e.g., maps, GPS, mileage calculator). Grand prize winner and guest must travel together on the same itinerary. Guest must be (i) at least 21 years of age or older or (ii) the child/legal ward of the grand prize winner. Winner and guest must possess any necessary valid travel documents (e.g., valid government-issued photo ID) and be available to travel between December 1, 2017 and December 1, 2018 or prize will be forfeited (at Sponsor's final discretion). Air travel will be from a major domestic airport near winner's place of residence as provided on the Entry form. Sponsor will determine airport, airline, flight itineraries, and hotel accommodations in its sole discretion. No refund or compensation will be made in the event of the cancellation or delay of the flight. No refunds or credit for changes are allowed. All other expenses and costs not expressly listed above are the winner's sole responsibility. Travel restrictions may apply. The actual value of the grand prize is expected to vary based on the point of origination, airfare fluctuations, and hotel pricing. If the actual value of the travel portion is less than stated, the difference will not be awarded in cash, and no such claim will be honored.
Additional Prize Details. All taxes, fees, and surcharges on prize are the sole responsibility of the winner. For gift card, terms and conditions may apply. For prizes over $600, the winner will be issued an IRS Form 1099 for the fair market value of the prize inclusive of sales or other transaction taxes. Affidavit of eligibility (including SSN), prize acceptance form, tax information, license agreement, publicity release, and/or mutually acceptable liability releases may be required where not prohibited. Any and all guarantees and warranties for merchandise prizes are subject to the manufacturer's and/or service providers terms and conditions, and winner agrees to look solely to such manufacturers and/or service providers for any warranty or guarantee claim. Prizes are non-transferable except in the sole discretion of the Sponsor. No substitution or cash equivalent is allowed. Sponsor reserves the right to substitute prizes of the same approximate retail value. Prize consists only of items specifically listed as part of the prize. All costs and expenses not specified herein, related to any prize, including but not limited to delivery, installation, service activation fees, and other expenses incurred by accepting the prize, are the sole responsibility of the winner.
LICENSE TO ENTRY MATERIAL: As a condition of entry, each entrant grants Sponsor a perpetual, irrevocable, royalty-free, fully sub-licensable and transferable worldwide exclusive license and permission to use, reproduce, edit, modify and exhibit, create derivative works of, transmit, distribute, and display the entry, in any and all media formats, whether now known or hereafter developed. Entrant will have no right of approval, review, or modification as to the future use, modification, or display of entrant's entry. Entrant shall have no right of compensation from, credit or attribution, or approval over any use of Entry by Sponsor. Sponsor has the right but not the obligation to post any entry for worldwide viewing on the Internet. Entrants hereby forever waive and relinquish all so-called "moral rights (droit moral)" now or hereafter recognized in connection with any entry submitted as part of the Contest. Entrants agree not to issue any publicity concerning Sponsor. Entrants agree that neither Sponsor, nor its agents, shall be responsible for return or preservation of the Entries submitted. All entries that are posted on Sponsor's website or elsewhere may be available to be viewed by the public.
PUBLICITY RELEASE: By entering, entrants authorize the Sponsor to post their entries on the Sponsor's webpage, #AmputeeLifeHacks contest site and the Sponsor's social media channels for viewing by a world-wide audience. Except where prohibited, acceptance of a prize constitutes winners' consent that his or her name, likeness, voice and/or biographical data may be used for advertising and promotional purposes without limitation and without additional notice, compensation, or consent. Submission of content to be posted online constitutes permission for same.
RELEASE: By entering, entrant agrees to accept and abide by the Official Rules of this Contest and agrees that any dispute with regard to the conduct of this Contest, rule interpretation, or award of prize shall be submitted to Sponsor, whose decision shall be binding and final. By participating, you release Sponsors, YouTube, Facebook, and their respective subsidiaries, affiliates, partners, employees, directors, and agents from any and against any and all injury, loss or damage including but not limited to, personal injury, death or damage to or loss of property, caused or claimed to be caused by your participation in the Contest. Entrant hereby agrees to defend, indemnify and hold harmless Sponsors and their respective officers, directors, employees, agents, parent companies, affiliates, subsidiaries, licensees, successors, assignees, against any and all third party claims, damages, liabilities, costs and expenses, including reasonable legal fees and expenses, arising out of or related to any breach by the entrant of any provision of this document, including, without limitation, Entrant's representations and warranties.

If for any reason Sponsor is prevented from continuing with this Contest, or the integrity and/or feasibility of the Contest is severely undermined by any event including but not limited to fire, flood, epidemic, earthquake, explosion, labor dispute or strike, act of God or public enemy, satellite or equipment failure, riot or civil disturbance, war (declared or undeclared), terrorist threat or activity, or any federal, state or local government law, order or regulation, order of any court or jurisdiction, Sponsor shall have the right, in its sole discretion, to abbreviate, modify, suspend, cancel or terminate the Contest without further obligation. If Sponsor in its discretion elects to so abbreviate the Contest, Sponsor reserves the right, but not the obligation, to award the prize from among all eligible entries received to date. All entries are the physical property of Sponsor and are not returnable.
DISPUTES / CHOICE OF LAW: Except where prohibited, each entrant agrees that: (1) any and all disputes, claims and causes of action arising out of or connected with this CONTEST or any prize awarded shall be resolved individually, without resort to any form of class action, and exclusively by state or federal courts situated in TEXAS, (2) any and all claims, judgments and awards shall be limited to actual out-of-pocket costs incurred, but in no event attorneys' fees, (3) no punitive, incidental, special, consequential or other damages, including, without limitation, lost profits, may be awarded (collectively, "Special Damages"), and (4) entrant hereby waives all rights to claim Special Damages and all rights to such damages multiplied or increased. TEXAS law, without reference to choice of law rules, governs the CONTEST and all aspects related thereto.
